Stage 4: Core therapy and stabilization

Stage 5: Discharge planning and continuity of care
- Discharge planning and post-treatment steps are prepared individually and coordinated by the clinical team. The goal is to support continuity of care, maintain clinical stability, and assist the patient in the next phase of treatment or therapy after discharge.
