What is binge eating disorder (BED)
Binge eating disorder (BED) is a recognised eating disorder characterised by recurrent episodes of consuming very large amounts of food in a short period, combined with a sense of loss of control. Unlike bulimia, binge eating episodes are not accompanied by regular compensatory behaviours such as self-induced vomiting, laxative misuse or excessive physical activity. The disorder is psychological and behavioural in nature, not merely dietary.
When urgent medical attention is required
Urgent medical attention is required if a psychological crisis arises involving suicidal ideation, intent to self-harm, loss of contact with reality, symptoms of acute psychosis, severe disorientation, rapid escalation of anxiety or agitation, aggressive behaviour disproportionate to the situation, or a state in which the patient is unable to ensure their own safety. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112 or 999.
BED as a psychiatric disorder
Binge eating disorder has been classified as a distinct diagnostic entity in both ICD-11 and DSM-5. It is not a matter of willpower, discipline or a consequence of overweight. BED is associated with impaired emotional regulation, impulsivity, brain reward mechanisms and frequently co-occurring depression, anxiety or trauma.

Health consequences of untreated BED
Untreated BED can lead to serious consequences including metabolic disorders (insulin resistance, type 2 diabetes), cardiovascular disease, sleep disturbances and chronic fatigue, diminished self-esteem and social isolation, depression, anxiety disorders and passive suicidal ideation, and co-occurring behavioural or substance addictions. The disorder is relapsing in nature and typically worsens without treatment.

Diagnostics and clinical assessment
The treatment process begins with a comprehensive assessment including psychiatric consultation, assessment of binge eating patterns and emotional regulation, diagnosis of co-occurring disorders, assessment of somatic and metabolic status, and analysis of triggering and sustaining factors.
Integrated BED treatment model

Stage 3: Aftercare planning
Preparation of an individual plan for continuation of treatment and support after the inpatient stay.
When inpatient BED treatment is indicated
Inpatient treatment may be appropriate when binge episodes are frequent and difficult to control, when loss of control persists despite outpatient treatment attempts, when co-occurring depressive, anxiety or post-traumatic disorders are present, when impulsivity or self-destructive behaviours are present, when BED significantly impairs professional or relational functioning, or when the patient requires structure and temporary separation from triggers.
A different level of care is required when there is an acute life-threatening condition, when the problem is mild and responding well to outpatient therapy, or when there is no readiness for inpatient treatment. Clinical qualification always precedes admission.
