What is codeine

Codeine is an opioid analgesic and antitussive medication belonging to the group of weaker opioids. In the body, it is partially converted to morphine - which is responsible for its pain-relieving and euphoric effects.

Because codeine is perceived as a "mild" medication, the risk of addiction is significantly underestimated - both by patients and those around them. In clinical practice, codeine can lead to the development of tolerance, physical dependency and psychological addiction, particularly with long-term use or when taken in excess of recommended doses.

When urgent medical attention is required

Urgent medical attention is required if any of the following occur during withdrawal: seizures, loss of consciousness, severe disorientation, symptoms of psychosis, aggressive behaviour disproportionate to the situation, severe chest pain, shortness of breath or suspected overdose. In these situations, no attempt should be made to manage the condition independently. Patient safety and rapid clinical assessment are always the priority. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112 or 999.

Codeine on prescription and outside medical control

Codeine is present in analgesic preparations, cough medicines and compound products - often combined with paracetamol or other substances. For some patients, the problem begins with legitimate use and then progresses to increasing doses, more frequent intake, combining with other medications or alcohol, and seeking preparations outside medical supervision.

How codeine addiction develops

The most common clinical pattern involves using codeine as directed, gradual development of tolerance, shortening of the duration of effect and the need for the next dose, emergence of withdrawal symptoms and compulsive use despite health consequences. Codeine addiction often develops imperceptibly, without classic "drug-seeking" behaviours - which delays diagnosis.

Untreated codeine addiction can lead to full opioid withdrawal syndrome, depression and anxiety disorders, liver damage - particularly with paracetamol-containing preparations - escalation to stronger opioids, and deterioration in cognitive and social functioning.
