What is zolpidem

Zolpidem is a sleeping medication from the class known as Z-drugs, used in the short-term treatment of insomnia. It acts by modulating GABA-A receptors in the central nervous system, producing a sedative and sleep-inducing effect. Although zolpidem is not a benzodiazepine, its mechanism of action affects the same brain structures responsible for sleep, relaxation and anxiety inhibition. For this reason the medication is not intended for long-term use.

When urgent medical attention is required

Urgent medical attention is required if any of the following occur during withdrawal: seizures, loss of consciousness, severe disorientation, symptoms of psychosis, aggressive behaviour disproportionate to the situation, severe chest pain, shortness of breath or suspected overdose. In these situations, no attempt should be made to manage the condition independently. Patient safety and rapid clinical assessment are always the priority. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112.

Why zolpidem can lead to dependency

Zolpidem dependency can develop even when the medication is used as directed, particularly when therapy continues for longer than a few weeks, doses are gradually increased, or the medication is taken daily as the only means of achieving sleep. Over time, the body develops tolerance, causing the sleep-inducing effect to weaken and creating a need for ever-higher doses. Psychological dependence can develop alongside this - a conviction that sleep without the medication is impossible.

How zolpidem addiction develops

The addiction process most commonly develops gradually: initially effective treatment of insomnia, followed by a shortening of the medication's duration of action, then increasing of the dose or frequency of use, anxiety and insomnia when cessation is attempted, and finally compulsive use of the medication despite awareness of the risks. For many patients the problem remains unrecognised for a long time because zolpidem is a prescription medication and is often perceived as "safe".

Symptoms of zolpidem addiction

Psychological symptoms

  • intense anxiety about not having the medication
  • insomnia that intensifies upon withdrawal
  • irritability and low mood
  • difficulties with concentration and memory
  • psychological dependence on the ritual of taking the medication

Physical symptoms

  • headaches
  • dizziness
  • sensation of disorientation
  • daytime fatigue
  • withdrawal symptoms after a missed dose

Risks of long-term zolpidem use

Chronic misuse of zolpidem can lead to deterioration of sleep quality despite continued use, impairment of memory and cognitive function, episodes of amnesia and automatic behaviour, worsening of anxiety and depressive symptoms, and increased risk of accidents and injuries. When combined with alcohol or other sedatives, the risk of complications increases significantly.

When inpatient treatment is required

Inpatient treatment may be indicated when attempts at independent cessation result in intensified symptoms, there has been prolonged daily use, psychological or neurological symptoms are present, outpatient treatment has been ineffective, or other addictions or psychiatric disorders are co-occurring. Every case requires individual clinical assessment.

Treatment of zolpidem addiction does not consist of replacing one medication with another, but of the gradual stabilisation of the patient's psychological state and the restoration of physiological sleep mechanisms. Abrupt cessation of zolpidem can be associated with significant medical risk. In cases of severe symptoms or risk to health, the patient may be referred to hospital treatment at a higher level of care.

When inpatient zolpidem addiction treatment is indicated

For whom inpatient treatment may be appropriate

  • long-term users of zolpidem, including those who have used it in accordance with the original prescription
  • patients with increasing tolerance requiring dose escalation
  • those experiencing rebound insomnia, anxiety or agitation upon withdrawal
  • patients with loss of control over medication use
  • those with co-occurring anxiety or depressive disorders
  • patients for whom attempts at dose reduction in outpatient settings have been unsuccessful

When another level of care is required

  • severe neurological or psychiatric complications requiring acute hospitalisation
  • absence of patient consent to inpatient treatment
  • need for immediate treatment at a higher level of care

Scope of treatment and clinical responsibility

Residential treatment focuses on medical stabilisation, evaluation of the patient's psychological state and intensive therapeutic work in the acute or advanced phase of the disorder. It does not replace long-term outpatient treatment and does not guarantee any specific clinical outcome. Planning of further care is an integral part of the therapeutic process. Is zolpidem addictive?
Yes. Zolpidem can lead to both psychological and physical dependence, particularly with long-term use.
Can zolpidem be safely stopped without medical support?
This is not recommended. Cessation should always take place under medical supervision, as abrupt withdrawal can cause rebound insomnia, anxiety and in some cases more serious neurological symptoms.
Can someone become dependent on zolpidem even if they take it as prescribed?
