What is prescription fentanyl
Fentanyl is a powerful synthetic opioid used in medicine for the treatment of severe pain, including cancer pain or pain refractory to other treatment. In clinical settings it is available in several forms: transdermal patches, nasal or sublingual/buccal preparations, and solutions used in anaesthesiology.
Due to its exceptionally high potency, fentanyl carries a significant potential for the development of tolerance, physical dependence and psychological addiction - even when used as directed.
When urgent medical attention is required
Urgent medical attention is required if any of the following occur during withdrawal: seizures, loss of consciousness, severe disorientation, symptoms of psychosis, aggressive behaviour disproportionate to the situation, severe chest pain, shortness of breath or suspected overdose. In these situations, no attempt should be made to manage the condition independently. Patient safety and rapid clinical assessment are always the priority. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112 or 999.
Prescription fentanyl and other opioids - a clinical distinction
Fentanyl differs from other prescription opioids (such as oxycodone or tramadol) primarily in its considerably greater potency, faster onset of action at opioid receptors, higher risk of respiratory depression, and a clinically more demanding and complex withdrawal process. For these reasons, addiction to prescription fentanyl requires particular clinical caution and inpatient treatment.
How prescription fentanyl addiction develops
The most common clinical course involves use of fentanyl as directed for severe or chronic pain, gradual development of tolerance and shortening of the duration of effect, escalating fear of pain and withdrawal symptoms, compulsive use or dose manipulation, and loss of control despite awareness of the risk. In many patients the boundary between pain management and addiction becomes progressively blurred.
