Nitrous oxide (N₂O) addiction is a serious substance use disorder that can lead to severe neurological, psychological and physical damage. Although it is often perceived as a safe or recreational substance, prolonged or intensive use carries a real risk to health and life. Treatment requires inpatient medical care, neurological assessment and addiction therapy.

What is nitrous oxide
Nitrous oxide (N₂O) is a gas used legally in medicine, including anaesthesiology and dentistry, and in the food industry. In non-medical contexts it is sometimes used recreationally through inhalation, most commonly from cartridges or balloons.
In non-medical use, it is not administered in controlled doses, it is often used without supervision, it is sometimes combined with alcohol or other substances, and it carries a high risk of neurological complications.
Why nitrous oxide is dangerous
Nitrous oxide deactivates vitamin B12, disrupts nerve conduction, affects the nervous system, spinal cord and brain, and can cause permanent neurological damage. Regular and prolonged use is particularly dangerous and can lead to irreversible changes even in young, previously healthy individuals.
When urgent medical attention is required
Urgent medical attention is required if any of the following occur during or after use: seizures, loss of consciousness, severe disorientation, symptoms of psychosis, aggressive behaviour disproportionate to the situation, severe chest pain, shortness of breath or suspected overdose. In these situations, no attempt should be made to manage the condition independently. Patient safety and rapid clinical assessment are always the priority. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112.
How nitrous oxide addiction develops
The addiction is primarily psychological and behavioural in nature, but its consequences are serious on a physical level. The mechanism involves a rapid, short-lived sensation of euphoria or dissociation, a desire to repeat the experience, a gradual increase in the frequency of use, the development of tolerance and a loss of behavioural control. For many patients, nitrous oxide becomes a way of regulating anxiety, boredom, tension or psychological discomfort.

Health risks and complications
Untreated nitrous oxide addiction may lead to permanent damage to peripheral nerves, myelopathy affecting the spinal cord, encephalopathy, sensory and motor disturbances, lasting disability and deterioration of mental health. Some changes may be irreversible if treatment is delayed.
Why inpatient treatment is indicated
Outpatient treatment is often insufficient because of the need for urgent neurological assessment, the need for supplementation and metabolic stabilisation, the risk of further damage to the nervous system, and co-occurring psychiatric disorders or other dependencies.

Clinical assessment
The initial assessment includes a detailed medical and neurological history, psychiatric consultation, laboratory tests including vitamin status, and evaluation of the degree of nervous system damage.
Detoxification and stabilisation
The process includes complete cessation of the substance, symptomatic treatment, supplementation including vitamin B12 where indicated, and monitoring of neurological status.
Addiction therapy
The therapeutic programme includes individual psychotherapy, work on compulsive mechanisms, psychoeducation on relapse risk and planning of further treatment.
Stages of treatment

Detoxification following nitrous oxide use is carried out in inpatient conditions under medical and psychiatric supervision.
