What are anxiety disorders
Anxiety disorders are a group of psychiatric conditions characterised by excessive, chronic or episodic anxiety that is disproportionate to the situation and significantly disrupts everyday functioning. This anxiety does not serve an adaptive function and persists in the absence of real danger. Anxiety disorders are not a sign of psychological weakness or a personality problem - they are recognised medical conditions with confirmed neurobiological mechanisms requiring professional diagnosis and treatment.
When urgent medical attention is required
Urgent medical attention is required if a psychological crisis arises involving suicidal ideation, intent to self-harm, loss of contact with reality, symptoms of acute psychosis, severe disorientation, rapid escalation of anxiety or agitation, aggressive behaviour disproportionate to the situation, or a state in which the patient is unable to ensure their own safety. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112 or 999.
Neurobiological and psychological mechanisms of anxiety
Anxiety disorders are associated with dysregulation of the systems responsible for the stress response, including the HPA axis and structures such as the amygdala and prefrontal cortex. This can lead to a persistent state of tension and hypervigilance, misinterpretation of stimuli as threatening, difficulty extinguishing fear responses and a reduced tolerance for stress and uncertainty. Psychologically, anxiety is frequently reinforced by avoidance, excessive control, perfectionism and previous traumatic experiences.
Clinical terminology
The term 'neurosis' is used in everyday language, however current medical classifications (ICD-11, DSM-5) use the term anxiety disorders. On this page we use contemporary clinical terminology, referring to various forms of pathological anxiety requiring treatment.

When inpatient anxiety disorder treatment is indicated
Inpatient treatment may be appropriate for individuals with severe or chronic anxiety symptoms, frequent panic attacks, significant functional impairment, insomnia or somatic symptoms, co-occurring depression, PTSD or addiction, ineffective outpatient treatment, or those requiring therapeutic structure and continuous clinical supervision.
A different form of care is required when there is an acute life-threatening condition, when closed psychiatric hospitalisation is necessary, or when the patient is seeking only a brief outpatient consultation. Every decision is based on individual clinical assessment.
