What are synthetic cannabinoids (Spice, K2)
Synthetic cannabinoids are a group of illegal, laboratory-modified chemical substances that act on cannabinoid receptors in the brain, often many times more powerfully than THC. Despite colloquial names such as "synthetic marijuana", these substances are not marijuana. They have unpredictable compositions, often contain mixtures of multiple compounds, and are not subject to any quality control. Their clinical effects are far more toxic and dangerous than those of natural cannabis.
When urgent medical attention is required
Urgent medical attention is required if any of the following occur during withdrawal: seizures, loss of consciousness, severe disorientation, symptoms of psychosis, aggressive behaviour disproportionate to the situation, severe chest pain, shortness of breath or suspected overdose. In these situations, no attempt should be made to manage the condition independently. Patient safety and rapid clinical assessment are always the priority. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112.
Why synthetic cannabinoids are exceptionally dangerous
Unlike THC, synthetic cannabinoids strongly and persistently activate CB1 receptors, can act neurotoxically, frequently trigger acute psychotic disturbances and cause severe somatic reactions. The composition of any given batch can vary dramatically, making the risk of overdose and sudden complications extremely high.
How Spice and K2 addiction develops
Addiction can develop very rapidly, sometimes after just a few uses. The mechanism involves intense stimulation of the reward system, rapid disorganisation of emotional regulation, rapid development of tolerance and severe psychological symptoms when cessation is attempted. For many patients, synthetic cannabinoids become the primary means of coping with tension, anxiety or emotional emptiness.

Use of synthetic cannabinoids is associated with the risk of acute psychosis requiring hospitalisation, permanent psychiatric disorders, nervous system damage, rhabdomyolysis and renal failure, and sudden death. Clinical literature also documents deaths in young people with no prior medical history.
Why inpatient treatment is indicated
In the case of substance use disorders involving synthetic cannabinoids, outpatient treatment is in most cases insufficient and carries a high risk. Treatment is therefore provided within a specialist drug addiction treatment programme under inpatient conditions. Inpatient treatment enables an immediate response to psychosis, round-the-clock medical supervision, safe symptomatic treatment, separation from the substance, and neurological and psychiatric assessment.

Stage 1 - Clinical assessment
Urgent psychiatric consultation, assessment of psychosis and self-harm risk, differential diagnosis of intoxication versus psychiatric disorder, and laboratory and neurological investigations where indicated.
Stage 2 - Detoxification and stabilisation

Scope of treatment and clinical responsibility
Residential treatment focuses on medical stabilisation, evaluation of the patient's psychological state and intensive therapeutic work in the acute or advanced phase of the disorder. It does not replace long-term outpatient treatment and does not guarantee any specific clinical outcome. Planning of further care is an integral part of the therapeutic process.
