Clinical picture of bulimia nervosa
Bulimia nervosa is a recognised eating disorder characterised by recurrent episodes of binge eating followed by compensatory behaviours such as self-induced vomiting, laxative misuse, periodic starvation or excessive physical activity. Unlike anorexia, the body weight of individuals with bulimia often falls within the normal range, which can delay recognition. Despite this, bulimia is associated with significant medical and psychiatric risk and frequently co-occurs with depression, anxiety disorders, trauma, impulsivity and addictions.
When urgent medical attention is required
Urgent medical attention is required if a psychological crisis arises involving suicidal ideation, intent to self-harm, loss of contact with reality, symptoms of acute psychosis, severe disorientation, rapid escalation of anxiety or agitation, aggressive behaviour disproportionate to the situation, or a state in which the patient is unable to ensure their own safety. In any life-threatening or health-threatening situation, emergency services should be contacted immediately by calling 112 or 999.
Medical and psychological risks of bulimia
Untreated bulimia can lead to serious complications including electrolyte disturbances and dehydration, dangerous cardiac arrhythmias, gastrointestinal and oesophageal damage, enamel erosion and oral infections, metabolic instability, and impulsivity, emotional dysregulation and risk of self-harm. Somatic deterioration can progress rapidly, particularly when binge-purge cycles are concealed.

Bulimia treatment is delivered within a comprehensive eating disorder treatment framework in a confidential inpatient model, with an emphasis on round-the-clock medical supervision, psychiatric responsibility, individual therapy planning and structured, non-stigmatising therapeutic care. An interdisciplinary team of physicians, psychiatrists, psychologists and psychotherapists experienced in treating complex eating disorders provides ongoing care.
Comprehensive clinical assessment
Each patient undergoes detailed diagnostics which may include an internal medicine examination and laboratory investigations, psychiatric consultation, nutritional and metabolic assessment, analysis of the frequency and severity of binge-purge episodes, and diagnosis of co-occurring disorders including addictions. On this basis, an individual, flexible treatment plan is created.
Integrated bulimia treatment model
Treatment addresses both somatic safety and the psychological mechanisms of the disorder and may include medical stabilisation and electrolyte monitoring, restoration of regular meal patterns, individual psychotherapy, psychiatric support where clinically justified, work on emotional regulation and impulsivity, psychoeducation about the binge-purge cycle, and supportive therapies where indicated. The aim of treatment is lasting behavioural change and relapse prevention.
Stages of bulimia treatment
Stage 1: Medical stabilisation
Correction of electrolyte disturbances, reduction of somatic risk and interruption of the binge-purge cycle under clinical supervision.
Stage 2: Targeted therapy
Work on emotional regulation, impulsivity, shame, perfectionism, trauma and cognitive distortions relating to food and body image.
Stage 3: Continuity of care
Discharge planning, relapse prevention and coordination of further outpatient or specialist treatment.
When inpatient bulimia treatment is indicated
Inpatient treatment may be appropriate for individuals with recurring binge-purge episodes, entrenched bulimic behaviours, somatic complications, high impulsivity or co-occurring psychiatric disorders, or cases of ineffective outpatient treatment.
Inpatient treatment may not be appropriate where immediate internal medicine hospitalisation is necessary, where the primary need is short-term dietary support, where there is no readiness for inpatient treatment, or for those seeking only dietary counselling without clinical care. Every case is individually qualified.
